August 2016 – present: Medical-Surgical and Swing Bed RN Case Manager – Red Bud Regional Hospital, Red Bud, IL
Responsibilities include, but not limited to: utilizing Interqual criteria to determine pt appropriate level of care: Observation vs Acute, admitting both Acute level of care and Swing Bed/SNF level of care patients, conducting patient care/family conferences to assist team with appropriate plan of care, discharge planning including patient/family education, maintaining good working relationship with other agencies in the community, such as Home Care agencies and surrounding hospitals, and assisting with obtaining insurance authorization for hospital stays for acute level and SNF level of care patients.
March 2014 – May 2016: Medical-Surgical RN Case Manager – Effingham Hospital, Springfield, GA
Responsibilities include, but not limited to: assisting the Hospitalist physician on duty with assessing patients for possible admission, utilizing Interqual criteria to determine patient’s appropriate level of care: Observation vs Inpatient, covering Swing Bed program, discharge planning including patient/family education, maintaining good working relationship with other agencies in the community, such as Child Protective/Adult Protective Services and Home Care Services.
October 2012 - June 2013: Emergency Room RN Case Manager – Nicholas H. Noyes Memorial Hospital, Dansville, NY
Responsibilities included, but not limited to: assisting the Emergency Room physician with assessing the pt for possible admission, utilizing Interqual criteria to determine pt appropriate level of care: Observation vs Inpatient, instructing Emergency Room nursing staff and physicians on criteria for admission, discharge planning including pt /family education, maintaining good working relationship with other agencies in the community, such as Child Protective/Adult Protective Services and Home Care Services.
Jan 2011 - June 2013: RN Case Manager - Nicholas H. Noyes Memorial Hospital, Dansville, NY
Responsibilities included, but not limited to: directing patient care, utilizing Interqual admission/observation criteria to determine new admission’s appropriate level of care, updating physicians and insurance companies on patient status, discharge planning, initiating the case management process with scheduled orthopedic surgery patients prior to admission (ie: Total Joint Replacements), pt/family education, maintain good working relationship with vendors in community.
May 2009 - June 2010: RN Case Manager - Christiana Care VNA, Camden, DE
Responsibilities included, but not limited to: admission and discharge of patients using the computerized OASIS forms, pt/family education, direct patient care, supervision of support staff, updating insurance companies and physicians of patient progress, discharge planning.
